Dear XXXX, 


You have tested positive for COVID-19. Please read this whole message. There are steps you must take. 

We know this is a difficult time and medical and other help is available. [For more languages: or call 1300 651 160 for an interpreter] 



In order to help us protect others and provide the best support for you and your family, we need you to answer these questions: It takes 1 minute.



  1. Focus on your health and get help if you need it. To see how to manage COVID-19 at home, and when you should go to hospital, read 
  2. You must immediately isolate at home for 7 days from the date of your first positive test. You can leave isolation on 15/02/2022. You can leave for medical goods or if there is an emergency such as going to hospital or escaping family violence. 
  3. You must tell your workplace, school, the people you live with, and friends and family you’ve been with recently that you have COVID-19. We will not do this for you. Some of your contacts (including everyone you live with) must get tested and isolate for 7 days. Follow this checklist: 
  4. Answer the questions above. Even if you don’t need help, answering the questions means we can help people who need it faster. 


If you are re-exposed to COVID-19 within 30 days of your positive test result, you do not need to quarantine or test again. You can attend work, education and sensitive settings within this 30 day period.
